A massive double cheeseburger and fries are presented to the camera.

A man in a hospital gown is presented with a massive double cheeseburger and fries. He struggles to eat it, then is playfully spanked by a woman in a nurse costume. The restaurant has a hospital theme, with nurses and medical imagery. The man is then shown weighing himself, and the text indicates that if he weighs over 350 pounds, his meal is free. He weighs in at 176 lbs, then 168 lbs, and finally 170 lbs, indicating he does not qualify for the free meal. He is then shown eating the burger and fries, feeling full, and getting spanked again.
